I used to have my trakball (c) as a foot pedal for the game Blood. It had an "alternate fire" method and I was still using the keyboard to exclusively move my dude around. So I had the trakball on the ground next to my foot and when I wanted to fire a weapon in its other mode I'd stab at it hoping to hit one of the big buttons on either side. (There used to be trakball choices other than the Kensington.)

I'm okay now, but it's my understanding that after this surgery such a method of communicating my desire to kill people over a LAN would be wholly unavailable to me.
